COBRAToday Providers Are Expected To
- Sign the Provider Agreement and return it with the appropriate fee. (Mail the Agreement to TASC, 2302 International Lane, Madison, WI 53704-3140 or fax it to 608-241-4584.) You will receive your Provider I.D. number and marketing materials in a matter of days.
- Assist Clients with completing the Plan Application. Accurate and complete Plan Applications are essential to properly installing of a COBRAToday Plan.
- Calculate the fee. Using the COBRAToday Fee Schedule, determine the enrollment fee for your Client’s group.
- Mail the Plan Application and Enrollment Fee to COBRAToday. Instruct your Client to mail the enrollment fee and the completed Plan Application. The Client kit, including all the necessary notification forms, will be sent to the group’s designated contact person once COBRAToday has received and reviewed the plan information.
