FlexSystem Providers are Expected To:
- Sign the Provider Agreement and return it with the appropriate fee. (Mail the Agreement to TASC, 2302 International Lane, Madison, WI 53704-3140 or fax it to 608-241-4584.) You will receive your Provider I.D. number and marketing materials in a matter of days.
- Assist Clients with completing the Plan Application. Accurate and complete Plan Applications are essential to properly install a FlexSystem Section 125 Plan. Work with your Clients to select the benefits they want for their employees, and to set the appropriate limits on excluded employees.
- Calculate the fee. Using the FlexSystem Fee Schedule, determine the enrollment fee for your Client’s group.
- Mail the Plan Application and enrollment fee to FlexSystem. Instruct your Client to mail the enrollment fee and the completed Plan Application. Employee Enrollment Kits will be sent to the group’s designated contact person once FlexSystem has received and reviewed the Plan information.
