AgriPlan Providers Are Expected To:
Sign the Provider Agreement and return it with the appropriate fee. Becoming a Provider of
AgriPlan is quick and easy. Just fill out and sign the Provider Agreement and fax or mail it to

53704-3140 or fax it to 608-241-4584.) You will receive your Provider I.D. number and marketing
materials in a matter of days.
Understand the Basic Concepts. Providers should understand the basic concepts surrounding the
Plan. The more a Provider understands, the easier it will be to explain to Clients.
Identify Clients who qualify for this tremendous tax savings. Sole Proprietors who can
legitimately employ their spouses in the business are perfect candidates for AgriPlan. Many
partnerships, corporations and limited liability companies may also be eligible.
Market AgriPlan to your Clients. You should utilize AgriPlan marketing support to get the word
out regarding AgriPlan. Samples of direct mail pieces, advertisements, and press releases are
available in the Provider Manual.
Enroll your Clients. We encourage you to play an active role in the enrollment process, as
well as in other areas involving your Clients. This may include the renewal process and
