LEGACY
guard Providers are expected to:
- Sign the Provider Agreement and return it with the appropriate fee. (Mail the Agreement to TASC, 2302 International Lane, Madison, WI 53704-3140 or fax it to 608-241-4584.) You will receive your Provider ID number and marketing materials in a matter of days.
- Assist Client with filling out the Plan Application. Accurate and complete Plan Applications are essential to properly install a LEGACY guard Asset Protection Plan.
- Calculate the fee. Using the LEGACY guard Fee Schedule, determine the enrollment fee for your Client.
- Mail the Plan Application and enrollment fee to LEGACY guard. Instruct your Client to mail the enrollment fee and the completed Plan Application. Client Kits will be sent to the designated contact person once LEGACY guard has received and reviewed the plan information.
- Help your Clients execute the Legal Documents. Once the documents are drafted to reflect the Client’s wishes, they must be signed, witnessed and notarized in order to become effective.
