To get started, simply download, print, and complete the Claim ConneX Plan Application, taking care to include thorough carrier and plan information; then sign and return the form to TASC. Upon receipt of the completed form, we will perform a number of tasks on your behalf to begin the process of activating this great benefit. We will contact the carrier(s),* obtain and implement the necessary agreements, and establish and test the Claim ConneX procedure for you!
