Administrative Forms

COBRA Premium Collection Form
To maintain compliance with federal law, TASC COBRA requires that any changes in rates must be submitted to TASC by the 15th of the month prior to the effective date.
Qualifying Event Notification
For fastest processing, submit this form online via support request.
Takeover Qualifying Event Notification
Data must be received by the 15th of the month prior if administration is to begin on the first day of the following month.

As part of our New Client Onboarding process, we offer a complementary training session. This interactive session helps ensure the success of your plan right from the start. It provides you with the opportunity to ask questions about your new plan and explains how to use many critical administrative features, including the Benefit Continuation Services website and much more.

If no available webinar works with your schedule, we will make every effort to accommodate an individual training session that does. To schedule an alternate session, please email tasconboarding@tasconline.com and provide two available dates and times. Once scheduled, an email will be sent to confirm your appointment.

Review Your Plan

It is important that you review your individual plans once your records are available. TASC requires that you audit the tiers and payment amounts (premium amounts) that are established and notify us immediately if any updates are required. To do so, sign into your account:

  • Go to ubaclient.tasconline.com
  • Before you can sign in for the first time, you must sign up for an account. Locate the area that asks “First time here?” and click the green “Sign up” link.
  • Enter your email address and create a password.
